I understand and acknowledge that as a member of the Rochedale Revue, my participation as part of the revue may be incorporated into a sound recording or film. I consent to the making of any sound recording or film of me/my child by representatives of the Rochedale Revue and authorise The Scout Association of Australia Queensland Branch Inc. to make use of such recording or film to do any of the Acts specified in Section 248G of the Commonwealth Copyright Act 1968 including, but not limited to:
1. Making a direct or indirect recording of any performance;
2. Broadcasting or re-broadcasting the performance or a recording of a performance; and
3. Causing the performance, or recording of the performance to be transmitted to subscribers to diffusion.

I hereby give my approval for my son/daughter to participate in the 2019 Rochedale Revue. I have read and hereby give my consent to the Consent of Use of Performance in relation to my son/daughter and give authority as per the following statement. I have read the Cast and Crew Application Information (documents tab of Revue App) and understand all the guidelines and information given. I agree to pay the activity fee of $100 (cast only) to assist with the production of the revue. In the event of injury to my son/daughter, where reasonable attempts to contact me are unsuccessful, I give authority for such medical treatment to be given to my son/daughter as is recommended by the Medical Practitioner and seems, in the opinion of the leader in charge, to be reasonable and appropriate. I undertake to be reasonable for any fees or charges with respect to that treatment and to pay those costs on demand by the Association.
